New approach to delivering health and social care services a significant step forward

The proposals are set out in the Welsh government’s long-term plan for the future of health and social care in Wales, A Healthier Wales, which focuses on providing more joined-up services, in community settings - removing many of the current frustrations expressed by those both using and working within the system.

The changes will begin immediately, with a £100 million transformation fund to support the implementation of the plan. The funding will be targeted at resources to speed up the process, including the development of new integrated prevention services and activities in the community.

This is a plan that if delivered, will help ensure that we begin to turn a corner on avoidable health inequalities. The quality, security and accessibility of our housing play a hugely significant role in our ability to maintain our quality of life, manage ill-health and feel positive about our environment. This plan recognises that housing has a substantial impact on health, but that housing, as a sector is having and can have a further impact in innovating how services are delivered for the better. We must continue to build momentum and look at enacting the practical steps that will ensure the ambitions outline in this plan become a reality across Wales.
